- comments: observed no valid city of chicago certified manager on duty while tcs food are being prepared, handled, and served such as ground beef. Priority foundation violation. Citation issued. 7-38-012.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed pink and brown slimy substance in the interior of the ice machine at the top of the ice shoot. Also noted slimy substance dripping unto the ice. Instructed to clean and sanitize the interior of the ice machine and maintain. Priority foundation violation. Citation issued. 7-38-005.
38. Insects, rodents, & animals not present
- comments: observed 35 mice droppings scattered on the floor under pallets and ledging in the rear storage area, and 25 mice droppings scattered on the floor inside the walk-in cooler. Also noted 15 mice droppings in one box of chips and one bag of chips gnawed into. Insturcted to remove droppings and detail clean all affected areas. Priority foundation violation. Citation issued. 7-38-020(a).

- comments: observed front door not tight fitting on the size or bottom. Instructed to repair and maintain tight fitting to prevent entry points for pests.

- comments: observed accumulation of dust on the fans in the walk-in cooler. Instructed to clean and maintain.

- comments: observed leak underneath the three compartment sink in the rear storage area. Instructed to repair and maintain.

- comments: must detail clean the walls, ceilings and floors inside the walk-in cooler especially where mice droppings were found.
