- comments: observed rear exit door leading to outside receptacle with at least 1/4 inch gap between door and floor. Instructed to rodent-proof door to prevent pest entry.

- comments: observed evidence of pest activity on premises during time of inspection. Noted approximatley 100 small rodent droppings throughout prep area behind heavy equipment, in manager's office, under dishmachine, under bulk container of oatmeal, on floor scattered along wall near ice machine, under shelving units throughout dry storage, and prep tables. Noted approximatley 100 small live black flies on walls, flying around near dish machine, on surfaces of 3 compartment sink, prep tables throughout, cooling units and ice machine. Recommended to contact pest control for additional service. Instructed to remove all droppings from said areas, and must eliminate pest activity. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: observed leaky piping on 3 compartment sink in rear prep near ovens causing water to pool on floor under said equipment. Noted hot water nozzle on faucet neck loose and leaking excessively. Instructed to repair equipment and maintain in good condition.

55. Physical facilities installed, maintained & clean
- comments: observed broken and missing floor tiles in front of dish machine and 3 compartment sink and entry space to prep area, noted missing grout throughout entire prep area. Instructed to provide smooth and easily cleanable flooring throughout said area and maintain.

- comments: observed previous core violations not corrected from report #2593249 on 4/23/24. They are as follows. . . 55)6-201. 13 observed broken wall base and detached base (black) throughout kitchen area. Instructed to repair and maintain to prevent breeding spaced for pest 55)6-501. 12 noted all floor drains in need of cleaning throughout kitchen, drain with excessive food debris buildup on floor near fryers. Instructed to remove spill on floor inside dry storage closet and remove pooled water underneath 3 compartment sink where chemicals are attached. 56)6-303. 11 observed burned-out light bulbs in the ceiling in the rear prep area near the 3-component sink. Instructed to replace and intensify lighting. Instructed to correct and maintain as required by code. Priority foundation violation #7-42-090. Citation issued.
