- comments: 6-301. 14 observed a hand washing sign is needed at the drink prep area hand washing sink. Instructed to provide.

- comments: 4-601. 11(c) observed accumulated food and grease debris located in the interior of the fryers, and under the cooking equipment. Instructed to clean.

- comments: 5-204. 12 unable to locate the back flow prevention device on the ice machine. Instructed to provide or locate.

- comments: 6-501. 12 observed a minor black substance on the walls in the dish washing area. Instructed to clean.
59. Previous priority foundation violation corrected
- comments: 8-404-13(b:3) observed previous priority foundation violations not corrected from inspection report #2583655 dated 10/19/23; 25 - 3-603. 11 observed items that can not be served undercooked or raw with consumer advisory disclosures on the menu such as, shrimp cocktail, crab louie, lobster items, scallops, seafood omelette, and paella a la valencia. Instructed to remove the disclosure for said items. Observed no consumer advisory disclosure for some items on the menu that can be ordered undercooked or raw such as beef and salmon that can be added to salad. Instructed to provide a consumer advisory disclosure for these items that can be ordered undercooked or raw. 38 - 6-501. 111 observed evidence of live insect activity on site. Observed about 50 live small flies on the wall behind the bar, observed about 10 live small flies on the ceiling and walls of the dish washing area, observed about 5 live small flies walls and ceiling of the drink prep area. Instructed to eliminate live small flies activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Instructed to correct said previous violations. Priority violation. Citation issued 7-42-090.
