Report date: Jan 30, 202438. Insects, rodents, & animals not present
- comments: observed evidence of rodent and live insect activity on the premises. Observed approx. 30 live ants in the lunchroom along the walls. Also observed over 100 mouse droppings in the following areas: 15 mouse droppings on the floor and storage shelves next to the mop sink and kitchen outside exit door, 10 droppings in the kitche area behind the coolers and ovens, 50 droppings under the 3 compartment sink and stoage shelves next to the sink, 30 droppings on the floor of the dish room, 10 droppings along the walls and corners of the lunchroom, and 30 droppings on the floor, storage shelves and cabinet under the sink in the principal's office. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the rodent and insect activity. Priority foundation violation 7-38-020(a).

- comments: seal the cracks between the floor and walls of the lunchroom to eliminate pest activity.

- comments: no working lighting is provided inside of the walk-in cooler. Must provide adequate lighting in the walk-in cooler.
