- comments: observed food employee handling ready-to eat food (burger buns) with bare hands. Food employee was assembling a cooked burger with bare hands. Instructed management that all ready-to eat foods must be handled with suitable utensils, deli tissue, or gloves. Priority violation 7-38-010. Citation issued.

- comments: prep area handwashing sink is missing handwashing signage. Must provide.
38. Insects, rodents, & animals not present
- comments: found over 50 rat droppings in the dry storage area and in the boiler room, both located in the basement. Instructed management to contact pest control for service to eliminate all pest activity from the premises and clean and sanitize affected areas. Priority foundation 7-38-020(a). Citation issued.

- comments: observed a large hole in a window in the adjacent building space that is currently unoccupied. Instructed management to rodent proof window, or keep the connecting door closed to minimize pest entry.

- comments: found heavy grease build up inside fryers. Must clean and maintain.

- comments: previous core violations from 1/12/2021 report 2472576 not corrected: violation #49 (4-602. 13) must detail clean loose food debris from food storage compartments (lower shelves) throughout food prep area. Instructed to maintain. Violation #55 (6-201. 11) must detail clean debris from floors under and behind heavy equipment in prep area, and basement storage area, walk-in cooler & freezer, and toilet rooms. Instructed to maintain. Violation #56 (4-202. 18) must detail clean grease accumulation & debris from filters in kitchen hood, instructed to maintain. Must correct previous core violations.
