- comments: inaccurate, broken internal thermometer inside the rear walk-in cooler. Exterior walk-in cooler thermometer not functional. Must replace internal thermometer or repair external thermometer.

- comments: all bulk foods such as flour and stored nuts throughout the basement must be labeled with the product common name.

- comments: the food handler at the front prep/display area noted handling foods without an effective hair restraint. Must provide.

- comments: wiping cloths throughout all food prep areas must be held between use in a container with a sanitizing solution.

- comments: raw wooden shelving at the sandwich prep station must be painted/sealed to be smooth and cleanable. ----must not use cardboard as liner for shelving throughout. ----storage shelving inside hallway closet with food debris. Must clean and maintain.

- comments: noted an excessive amount of water leaking from the rear kitchen large dish machine onto the floor. Must repair.

- comments: instructed to provide waste receptacles at all hand washing sinks using paper towels as hand drying devices.

- comments: noted cracked, damaged floor tiles with missing tile grout at the rear large dishwashing machine. Tiles with pooling liquid and food debris. Must repair/seal. -----water damaged ceiling tiles throughout the front and rear kitchen. Must replace. Wrapped ventilation above the front main kitchen food prep with torn, exposed insulation. Must replace/seal exposed insulation.
55. Physical facilities installed, maintained & clean
- comments: mops improperly stored at the service sink. Soiled mop heads must be stored inverted at the sink.
56. Adequate ventilation & lighting; designated areas used
- comments: employees must not store personal clothing/jackets in the food prep area. Must use the designated employee storage area provided.
60. Previous core violation corrected
- comments: previous core violations from report #2366095 on 3-20-20 not corrected. #53- no covered trash receptacles in washrooms. Must provide. #58- noted certified food manager on duty with no allergen training certification. Instructed that all food service managers are required to have the allergen training certification. Priority foundation violation.
