10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towel at the hand-sink in the prep area. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler obtained a paper towel for the hand sink during the time of inspection priority foundation violation #7-38-030(c). Citation issued.

22. Proper cold holding temperatures
- comments: observed time/temperature control for safety foods (10#'s raw pork at 58. 2 and tofu at 75. 2f) at improper cold holding temperatures ranging from 58. 2f - 75. 2f located in cold holding unit and on top of 3 compartment sink. Instructed to discard and denature all said tcs foods 20#'s with a value of 1,800. 00 and to maintain a cold holding temperature of 41�f at all times. Priority violation. 7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 30 or more rat and mouse droppings on floor around sump pump lid, on floor underneath wood stairs, and throughout the entire basement storage area. Must remove droppings, clean, and sanitize areas. Noted over 150 small black flies throughout front and rear prep, dining rooms, and basement area. Recommended to have a pest control operator to service premises on or after 9/16/25. (priority foundation 7-38-020(a)(citation issued)

39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the walk-in cooler, prep, and storage areas. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed eating utensils not inverted. Instructed management to avoid contamination by inverting utensils with handles exposed after sanitizing and air drying.

- comments: observed torn door gasket inside of the 2 door prep cooler. Noted gaskets on all cold holding encrusted with food debris instructed to replace rubber gaskets and deep clean others

- comments: observed water backing up at the 3-component sink area. When water is released from 2nd sink basin, water backs up into 1st and 3rd sink basins. Instructed manager to repair before re-inspection date. Priority foundation violation #7-38-025. Citation issued.

- comments: observed grease buildup on cooking equipment, walls, surrounding floors, and shelving units in the kitchen/prep areas. Instructed to degrease, clean, and sanitize.

- comments: observed grease spilled on the ground near the grease container. Instructed manager to clean and maintain the outside waste grease area and the waste grease container. Priority foundation violation #7-38-020(b). Citation issued.

- comments: observed the baseboards peeling off walls throughout rear prep area instructed to repair and maintain

Previous core violations not corrected: #51- 5-204. 12: **c** backflow prevention device, location (no backflow device) found no backflow prevention devices at mop sink faucet. Must provide where it can be serviced and located. #55 6-201. 11: **c** floors, walls, and ceilings- cleanability found walls in poor repair by hand sink at preparation areas. (chipping paint)must repair paint. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
