- comments: observed no employee holding a valid city of chicago foodservice manager certificate on site while tcs items like chicken were prepared. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no kit on site for responding to vomit and diarreha. Instructed to maintain kit with all necessary supplies on site at all times. Priority foundation violation 7-38-005. Citation issued.

- comments: observed handwashing sink in men's toilet room maintaining an improper temperature of 56. 8f. Instructed to repair or replace sink so that it is capable of delivering hot water under pressure between 100-120f. Priority violation 7-38-030(c). Citation issued.

- comments: observed cardboard boxes on floor of cook line being used to absorb grease and cling wrap on counter by warmer lights. Instructed to remove and maintain smooth, cleanable surfaces.

- comments: observed facility uses ddbsa sanitizer for pots and pans in 3 compartment sink but does not have ddbsa test strips on site at time of inspection. Instructed to provide ddbsa test strips. Priority foundation violation 7-38-005. Citation issued.

- comments: observed leak underneath sprayer sink located next to low temperature dish machine and sink coming off wall in bar. Instructed to repair and maintain in good condition.
