Report date: Apr 13, 202210. Adequate handwashing sinks properly supplied and accessible
- comments: found the rear cooks line handwashing sink closest to the chicken walk-in cooler without hot water; water temperature is at 87. 0 f. Must provide hot water at all handwashing sinks with a minimum temperature of 100 f and maximum of 120 f. Priority violation 7-38-030(c). Citation issued.
40. Personal cleanliness
- comments: observed a food handling employee preparing wings without a hair restraint. Management instructed that all food handling employees must wear a hair restraint while preparing food.
45. Single-use/single-service articles: properly stored & used
- comments: found boxes of single service items (to-go boxes) stored directly on the floor throughout the rear expo line area. Instructed management to elevate all single service articles off the floor and maintain.
49. Non-food/food contact surfaces clean
- comments: found broken glass and old spills inside the bottom of the reach in beer cooler in the bar. Instructed management to detail clean cooler and maintain.
51. Plumbing installed; proper backflow devices
- comments: found low water pressure at the bar area handwashing sink. Found the faucet handle on the left sink in the womens washroom broken. Instructed management to repair sinks and maintain.

- comments: found the right handwash sink in the mens washroom not maintained; sink is currently taped off and not being used; customers/employees instructed to use the left handwash sink that is operable and provides hot water. Instructed management to repair sink.
55. Physical facilities installed, maintained & clean
- comments: observed a large accumulation of trash, food debris/grease, and dirt on the floor in the following areas: underneath the three compartment sink in the rear dish area, underneath the wing fryers in the cooks line, and underneath the bar equipment. Also observed heavy ice build up on the floor inside the wing walk-in cooler. Instructed management to detail clean and maintain facility cleanliness at all times.
58. Allergen training as required
- comments: found multiple city of chicago certified food managers without an allergen training certificate. Instructed management that all employees who have a city of chicago food manager certificate must also complete allergen training.
