- comments: observed no city of chicago certified food manager on site while tcs foods were being prepared and served (chicken wings, beef burgers, salad). Instructed to have a certified food manager on site at all times while tcs foods are being prepared and served. Priority foundation violation. 7-38-012. Citation issued.

22. Proper cold holding temperatures
- comments: found cold tcs foods at improper cold holding temperatures inside the prep cooler located closest to the dish room. Grilled onion, cut tomatoes, shredded lettuce, pulled pork, cooked chicken, mac n cheese, garlic aioli, and guacamole found at temperatures ranging from 45f to 63f. Instructed to discard and denature said tcs foods and always maintain all cold holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued. Food discarded.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed improper ambient temperature inside of the prep cooler at 53f. Ambient temperature must be at 41. 0f or below inside all cold holding units. Priority violation. 7-38-005. Citation issued. Cooler tagged and may not be used until reinspected by cdph.

38. Insects, rodents, & animals not present
- comments: observed over 125 live fruit flies throughout establishment. Found approximately 75 in the 2nd floor kitchen, 25 in the rear hallway leading to the dumpster, 10 in the 1st floor mens washroom, 5 in the 1st floor womens washroom, and 10 in the 2nd floor bar. Must detail clean and remove all evidence of pest activity on premises, and contact pest control for additional service. Priority foundation violation. 7-38-020(a). Citation issued.

51. Plumbing installed; proper backflow devices
- comments: observed the third handwashing sink (out of 4) from the left in the mens washroom with hot water at 88f. Must provide hot water at 100-120f at all handwashing sinks.

58. Allergen training as required
- comments: observed no allergen training certificates on site. Instructed management that all city of chicago food managers must complete allergen training and keep certificates on site.
