- comments: observed facility food handlers preparing and handling�time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge of city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed the interior guard of the ice machine with some dirt, not clean. Instructed the person in charge to detail clean and maintain.
38. Insects, rodents, & animals not present
- comments: observed three live cockroaches on the front cover of the ice machine and two live cockroaches on the wall behind the ice machine in the basement. Instructed the person in charge to remove roaches, clean surfaces, and maintain premises free of pest. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a) citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored on the floor in the rear prep area. Instructed the person in charge to elevate all food six inches off the floor and maintain.

- comments: observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units. Instructed the person in charge to clean and maintain. (nonfood contact surfaces)

- comments: previous core violations from 06/16/2023 inspection, report #2577460 not corrected: #47- observed stained and deeply scratched cutting boards stored in kitchen dishwashing area. Instructed to replace and maintain. #47- observed skewers stored on the floor of the dishwashing storage area. Instructed to provide adequate shelving units where needed and store items at least six inches off floor to provide ease of cleanability. #51-observed no backflow device on the ice machine in the basement. Instructed to install backflow device on ice machine in basement. #53-observed no covered waste receptable in both women's washrooms. Instructed to provide. #55-observed floors in corners and behind equipment with debris and black buildup in kitchen prep and dishwashing areas. Instructed to detail clean and maintain. #56- observed dim areas in the large kitchen prep area near one compartment and handwashing sink. Instructed to install adequate lighting in kitchen prep area. Priority foundation violation 7-42-090. Citation issued.
