- comments: observed facility food handlers preparing and handling time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge of city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.

- comments: observed three live cockroaches on the wall behind the drying rack in the rear warewashing area. Instructed the person in charge to remove roaches, clean surfaces, and maintain premises free of pest. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed food handlers prepping food without hair restraints. Instructed person in charge to ensure that all food handlers wear hair restraints and maintain.
