- comments: found 14 live fruit flies in the back dishwashing area (8 on the back door and 6 on the low temperature dish machine). Instructed management to contact pest control for service to eliminate all pest activity from the premises. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed build up on the dishmachine trays that hold the dishes in place inside of the dishmachine. Instructed to clean trays to remove build up of lime, dirt, and mold-like substance or replace.
54. Garbage & refuse properly disposed; facilities maintained
- comments: found outside garbage dumpsters overflowing with garbage; trash, cardboard, food waste, and plastic preventing lid closure. Instructed manager trash must be stored inside garbage receptacle so that they are inaccessible to insects and rodents. Priority foundation violation. 7-38-020(b). Citation issued.
