- comments: observed no city of chicago certified food service manager on site while tcs foods are being hadnled prepared and served. Instructed above that city of chicago certified food manager has to be present at all times tcs foods area prepared and served. Priority foundation violation 7-38-012. No citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up procedure and supplies on the premises for vomit and diarrheal events. Instructed manager to provide procedure and vomit kit. Priority foundation violation 7-38-005. No citation issued.

- comments: observed west side doors not tighting observed one inch opening on bottom doors. Must make doors tight fitting to prevent pest entry.

- comments: observed low water pressure at the hand washing sink in the staff/employee washroom. Must repair and maintain.

- comments: no covered waste receptacle provided in women staff and girls washroom. Manager instructed to provide.
