- comments: observed waste water backing up from floor drain on to floor under 3 compartment sink on the south side of kitchen. Must repair to convey all waste water properly correctly. Observed leak at pipe (on left side) under 3 compartment sink on the north side of the kitchen. Must repair and maintain same. Priority violation 7-38-030(c). Citation issued.

- comments: must provide cover/lid for garbage in women's restroom.

- comments: observed previous core violations not corrected from report #2352333, dated 11/26/19. Must correct the following core violations: #38 observed the front entrance door to not be rodent proof with a gap in the middle of the door. Instructed to provide a seal on the front entrance door to be rodent proof. #40 observed a male employee with no hair restraint in the food prep area. Instructed to provide a hair restraint in all food prep areas. #55 observed the floor around the grease trap to be soiled with food splash and debris. Instructed to clean and maintain the floor around the grease trap. Priority foundation violation 7-42-090. No citation issued.
