- comments: no person in charge was present at the time of inspection. Priority foundation violation. No citation was issued.

2. City of chicago food service sanitation certificate
- comments: noted no certified food manager on duty while tcs foods (rice, chicken soups etc. ) were being prepared and served. Instructed management that a certified food manager must be present at all times tcs foods are prepared and served. Also, the person in charge must have a city of chicago certificate by the next inspection (upon being inspected) priority foundation violation 7-38-012. Citation issued

- comments: noted rusty and dirty shelves inside walk-in cooler and shelf by the dishwasher lined with foil paper at the kitchen prep area. Instructed to detail clean, remove foil paper and repaint with non-toxic food-grade paint and/or replace.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: noted no sanitizer test strip kit quat on site for the 3 compartment sinks at the time of inspection. Instructed to have a test strip kit on-site at all times to properly monitor the strength of the sanitizer. Priority foundation. 7-38-005

- comments: noted leakage at the bottom piping of the 3 compartment sink at the kitchen food prep area. Instructed to repair and maintain

- comments: noted washrooms on-premise with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women washrooms and unisex washrooms.

55. Physical facilities installed, maintained & clean
- comments: noted dirty and dusty ceiling fans throughout the kitchen prep areas. Instructed to clean and maintain.
