- comments: observed no one on site, in charge, at time of inspection.
2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site while time/temperature controlled foods are being prepared and served including beef and chicken patties. Priority foundation 7-38-012.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed warming unit located on side of bakery that does not have access to a handwashing sink. A handwashing sink with hot and cold running water must be installed in food dispensing area or the hot holding unit must be relocated to adjacent food preparation area that has access to handwashing. Priority foundation violation 7-38-030 (c). No citation issued at this time.
38. Insects, rodents, & animals not present
- comments: observed pest activity on the following areas: approximately 50 mouse/rodent droppings under 3 compartment sink, behind hot water heater behind stove; approximately 100 droppings on sides of ovens and proofers; approximately 50 mouse/rodent dropping in front service area under and behind hot holding; approximately 50 mouse droppings in rear dry storage under shelving; approximately 20 mice droppings in the men's washroom under sink. All mouse/rodent droppings must be removed. Observed the following conditions creating harborage including tools and excess equipment in rear dry storage and adjacent to men's washroom. Relocate all tools to a designated area away from food and remove all unnecessary equipment that is creating harborage. Priority foundation violation 7-38-020 (a).

- comments: observed floors very unclean under and behind stove and 3 compartment sink. Remove heavy grease accumulation under and behind stove. Floors need to be kept clean and dry. Observed floors of men's washroom unclean. Observed all floors need to be cleaned in detail throughout facility including rear dry storage, front food prep area, rear food prep area, service area in order to monitor pest control.

- comments: oberved cracked and broken floor tiles around catch basin under 3 compartment sink. Replace all cracked and broken floor tiles and maintain floor in a smooth and easily cleanable condition at all times.
