- comments: observed evidence of appx. 40 or more mice droppings on floor under both dish storage shelves,prep table,next to 2- door reach in cooler and storage closet in prep area. Must remove droppings. Clean and sanitize areas. Recommended to have a pet control operator to service premises on or after 9/22/20. (priority foundation 7-38-020(a)

- comments: must invert multi-use utensils on dish storage shelves.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must remove rust from walk in cooler shelving units. Must repair or replace worn door gaskets on 2- door reach in cooler.
51. Plumbing installed; proper backflow devices
- comments: must repair or replace 1 st fl. (girl's) faucet handle in poor repair and adjust faucet handles to stay on longer in 2nd fl. Staff toilet room.
