23. Proper date marking and disposition
- comments: observed ready-to-eat, time/temperature control for safety foods (tcs): cooked chicken and salsa held for over 24 hours without discard date. Instructed facility to provide discard date for any ready-to-eat tcs foods held for over 24 hours. Priority foundation violation. 7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed short 2 door reach in cooler located in rear prep at an improper cold holding temperature of 53f at time of inspection. Reach in cooler tagged and held for inspection by the chicago department of public health. Found no tcs food items located in said cooler. Only observed whole vegetables. Instructed not to hold any tcs food items in said cooler until it maintains a cold holding temperature of 41f or below and is re-inspected by the chicago department of public health. Priority violation. 7-38-005.

37. Food properly labeled; original container
- comments: found rice, flour, and beans taken out of its original container and stored in a bulk container. Must provide common name of ingredient.

38. Insects, rodents, & animals not present
- comments: found 1 live roach located in rice bulk dry storage bin, 3 roaches in food prep area and 4 roaches located on wall behind coolers in food prep. Must contact pest control. Priority foundation violation. 7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: found flour, herbs and other food items stored in food prep area without lid. Instructed manager to keep food covered when not in use to prevent miscellaneous sources of contamination.

49. Non-food/food contact surfaces clean
- comments: found outside of bulk food containers filled with food debris. Instructed manager to clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: found womens toilet room handwashing sink leaking from drain pipe. Must repair and maintain.

60. Previous core violation corrected
- comments: found continued non- compliance from inspection report #2567304 conducted on 11/18/2022. The following previous core violations were not corrected: 4-502. 13 observed cardboard being reused as a floor liner. Instructed not to reuse cardboard, all surfaces must be smooth and easily cleanable. 5-204. 12 unable to locate the back flow prevention device on the ice machine. Instructed to provide or locate. 6-501. 12 observed excessive accumulated debris around the grease trap located under the 3 compartment sink, behind and underneath all refrigeration units and shelves and under and behind all cooking equipment at the front food prep area. Instructed to clean and sanitize. 2-102. 13 observed no proof of food handler training for two female and one male food handlers. Instructed to meet requirement. Instructed manager to comply. Priority foundation violation. 7-42-090. Citation issued.
