9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: observed a food handling employee cutting and handling ready-to-eat food (green onions) with bare hands. Management instructed that no bare hand contact is allowed with ready-to-eat foods. Priority violation 7-38-010. Citation issued.

29. Compliance with variance/specialized process/haccp
- comments: observed a vacuum packaging machine in the basement food prep area and vacuum packaged foods (pork, beef, etc. ) being stored in the chest freezer and walk-in cooler without an approved haccp plan. Management instructed to have a cdph approved haccp plan before the using the vacuum packaging machine. Priority foundation violation 7-38-005. Citation issued.

32. Variance obtained for specialized processing methods
- comments: observed no approved cdph haccp plan on the premises to vacuum package meats. Management instructed to have an approved cdph haccp plan before using the vacuum packaging machine to package food under reduced oxygen. Priority foundation violation 7-38-005. See violation #29 above for citation.

38. Insects, rodents, & animals not present
- comments: observed approx. 15 live small flying insects on the boxes and shelves in the 1st floor food prep area and on the walls and ceiling of the basement food prep area. Additional pest control services is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

40. Personal cleanliness
- comments: observed multiple food handling employees working in the food prep areas without a hair restraint. Management instructed that all food handling employees must wear a hair restraint while prepaing food.

49. Non-food/food contact surfaces clean
- comments: clean the top of the convection oven in the 1st floor food prep area.
