- comments: 6-501. 112 observed approximately 45 dead cockroaches on the floor of the prep area, inside of containers used to store utensils and equipment, inside of takeout containers used to store food, behind and under the booths in the dining room, on the food prep tables and food prep equipment. Management instructed to clean and remove all dead insects throughout the premises and clean and sanitize all affected surfaces.

- comments: 4-602. 13 detail clean all cooking equipment, interior of oven, the steam table, wok station, microwave, storage shelves, interior of prep coolers, and interior of ice machine, prep tables and outside of storage containers for equipment and food.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 12 clean the grease on the floor beneath the oven. Clean the debris under the can rack and other equipment around the walls and in the corners. Clean the floor under the dining room booths.
