- comments: observed 12 live cockroaches in the oven, on the floor of the food prep area underneath the 3 compartment sink and exposed handwashing sink. Pest technician was onsite during the inspection for a pest inspection. Management instructed to eliminate all pest activity. Priority foundation 7-38-020(a). Citation issued.

- comments: clean all of the dead incects from the floor of the food prep area.
39. Contamination prevented during food preparation, storage & display
- comments: observed a bucket of rice being rinsed in the mop sink. Management instructed to use the 3 compartment sink for rinsing the rice. Priority foundation 7-38-005. Citation issued.

- comments: observed rust on the bottom interior plate of the 1 door prep cooler across from the wok station. Management instructed to remove the rust from the prep cooler surfaces.

- comments: detail clean all cooking equipment, the steam table, wok station, microwave, storage shelves, interior of prep coolers.

- comments: observed waste water backup from the exposed handwashing sink in the front food prep area and the 3 compartment sink. Observed waste water backing up from the floor drain and spilling onto the floor of the front food prep area while the water is running. Also observed waste water backup into the left and right compartments of the 3 compartment sink when water is drained from the middle compartment of the sink. Management instructed to repair and maintain the floor drain at the exposed handwashing sink and at the 3 compartment sink so that the water drains properly. Priority 7-38-030(c). Citation issued.

- comments: clean the grease on the floor behind the cooking equipment and on the wall behind the cooking equipment.

- comments: clean the excessive grease from the ventilation hood filters and the ventilation hood.

- comments: previous core violations not corrected from report #2293922 on 6/20/19: 39 - food containers stored on floor in prep, storage, and walk-in cooler/freezers. Must provide proper, raised shelving to store food containers 6' off floor. 41 - noted dirty wiping cloths on prep tables at front and rear prep areas. Not properly stored. Instructed properly to store all washcloths in a sanitizing solution especially when not in use. 43 - observed that utensils in use are stored in cold water, must stored utensils under running coldwater, under hot water (135 f and above)and in clean place protected from contamination. 49 - must clean food debris build up from fryer cabinet, wheel bases of all equipment. Must clean dust build up from glass shelving units in food storage area. 49 - must clean dirty shelving in walk-in cooler. 57 - observed no food handler's training certification from employee. Instructed that all employee must comply with food handler's training by the food handling regulation enforcement act. Priority foundation 7-42-090. Citation issued.
