1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic doesn't have a certified food managers certificate nor certificate posted to view must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no valid certified food managers certificate posted to view while tcs foods are being prepared handled and served such as(rice,chicken,beans,etc)must provide and maintain. (priority foundation 7-38-012)(citation issued)

16. Food-contact surfaces: cleaned & sanitized
- comments: observed improper sanitizing of multi-use utensils at 0. 0 ppm's by a male food handler. Observed male employee take a spoon to 3- compartment sink and rinse under running water and placed into a container of 7. 8 lbs of cooked honey chicken and another male dish washer was sanitizing at 0. 0 ppm's at 3- compartment sink he was washing and rinsing utensils and not sanitizing and placing utensils on dish storage shelves. Instructed to set up sink to properly sanitizing utensils. (priority 7-38-025)(cos)(citation issued)

- comments: observed evidence of appx. 25 or more live small flies on walls and inside of floor drain in unisex toilet room. Recommended to have a pest control operator to service premises on or after 4/25/25. (priority foundation 7-38-020(a)(citation issued)
