- comments: observed no city of chicago certified food service manager on site while tcs foods are being prepared and served such as rice, beans, beef pork etc. Instructed to have a city of chicago certified food service manager on site at all times food is being prepared and served. Priority foundation violation 7-38-012. Citation issued.

- comments: observed interior of ice machines located in the dish room and ice making machine unit above the soda dispensing system in dining area with excess black slime substance at the water reservior and panels. Ice is used for human consumption instructed to clean and maintain unit clean at all times to prevent contamination. Priority foundation violation 7-38-005 citation issued.
