- comments: observed grease on ground near grease container and leaking out of grease bin storage area. Instructed manager to clean and maintain outside waste grease area and waste grease container. Priority foundation violation #7-38-020(b). Citation issued.

- comments: observed storage area and enclosure for refuse, recyclables, or returnables littered with unnecessary items (ex. Pot of grease, trash, metal pipes). Instructed to remove and maintain a mentioned areas shall be maintained free of unnecessary items.
55. Physical facilities installed, maintained & clean
- comments: observed food debris and spills on the floors and surfaces throughout the facility. Instructed to detail clean under all shelving units, interior of prep dish area, around dish machine, prep tables and all cooking equipment to remove spills, food debris, dirt buildup and/or grease. Instructed to increase cleaning frequency and maintain at all times.
