- comments: dead roaches noted on glueboards. Instd to remove dead roaches, clean and sanitize affected areas.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: cardboard lining noted on shelves and alumunum foil on surfaces at interior of fryers and underneath flat top grill. Instd to remove cardboard and aluminum foil so as surfaces to be smooth, even and easily cleanable. Rusty surfaces noted on door handle of walk in cooler and underside of storage shelving in rear near warewashing area. Instd to remove rust and maintain. Wood shelves above 3-compartment sink in disrepair with worn and crumbling surface. Instd to repair/replace shelves and maintain.

- comments: the following not clean, excessive food debris/grease accumulation: interior cabinet of fryer, interior of all coolers including rubber gaskets of doors, all storage shelves, all legs, wheels of all equipment, and underneath flat top grill. Instd to clean and maintain same.
52. Sewage & waste water properly disposed
- comments: waste water backing up from floor drain and water leaking profusely from drain pipe when water draining from 3-compartment sink. Plumber arrived during inspection and repaired issue and 3-compartment sink properly draining 7-38-030(c) priority.

- comments: floor tiles missing and cracked at front prep area near chicken roasting equipment. Instd to repair and maintain. Ceiling in poor repair above pizza oven. Hole in wall near walk in cooler. Wall behind 2-compartment sink in disrepair, excessive caulking and covering on wall not tight fitting. Instd to repair and maintain same.
55. Physical facilities installed, maintained & clean
- comments: excessive clutter and unnnecessary items in outisde rear storage unit. Storage unit used for single service items, and other items. Unable to access entire unit due to excessive clutter. Instd to remove clutter, organize area and maintain to be able to access all areas.
