- comments: observed rodent/pest activity on premises. Found 150 mouse droppings on the floor and storage pallets in the basement area around under and behind equipment and on the 1st fl in the corners and along the wallbase. 60 small live flies on the walls, ceiling and storage racks in the basement and 1st fl and 1 large brown roach on the floor on the side of the oven 1st fl. Instructed pic to eliminate all activity, remove droppings, clean/sanitize all areas and have pest control company service prior to reinspection next week. Priority foundation violation 7-38-020(a) issued.
40. Personal cleanliness
- comments: observed employees not wearing hair restraints while in the prep area and handling food. Instructed pic to have all employees to wear a hair restraint that is in the prep area and handling food. Maintain same.
41. Wiping cloths: properly used & stored
- comments: observed wiping cloths not properly stored when not in use. Instructed manager to store wiping cloths in sanitizing solution (must be at the required ppm) when not in use. Maintain same.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed storage racks in the walk in cooler/freezer with rust. Instructed pic to remove rust and maintain rust free equipment.
51. Plumbing installed; proper backflow devices
- comments: observed a leak underneath the hand washing sink in the 1st fl rear prep area. Also, a leak under the middle compartment of the 3-compartment sink in the basement area. Instructed pic to fix and maintain all leaks.
60. Previous core violation corrected
- comments: observed the previous core violations from report #2578179 dated 07/03/2023 not corrected and/or maintained. Instructed pic to correct and maintain: -#49-instructed to clean the following: fan covers inside of walk-in coolers to remove dust. Exterior surfaces of floor mixers. -#55-instructed to clean floors and walls around under and behind cooking and other kitchen equipment. -#57-observed expired food handler certificates; instructed to have employees obtain valid certificates. Priority foundation 7-42-090 violation citation issued
