- comments: no procedure in place for responding to vomiting and diarrheal events. Instd to provide procedure and necessary supplies effective against norovirus. Priority foundation 7-38-005.
38. Insects, rodents, & animals not present
- comments: observe two live roaches crawling on floor underneth 3-compartment sink and one live roach on floor behind ice machine located in rear of premises. Additional pest control service is needed to eliminate roach activity. Priority foundation violation7-38-020(a). Citation issued.
51. Plumbing installed; proper backflow devices
- comments: observed leaking faucet basae at the 3-compartment sink in rear of premises. Must repair and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle provided in the women's washroom. Must provide.
55. Physical facilities installed, maintained & clean
- comments: must regrout floor throughout rear food preparation/dishwashing area and must maintain.

- comments: observed excessive dust build upon ceiling ventilation vents inside washrooms. Must clean and maintain.
