- comments: observed evidence of appx. 40 or more mice droppings on wall ledge at drive-thru,on service counter behind register,on top of soda boxes under front service counter,rear hallway outside of toilet rooms. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/9/25. (priority foundation 7-38-020(a)(citation issued)

- comments: previous core violations from report #2588228 1/18/24 not corrected #38-6-202. 15 : observed a gap along bottom of west outter door must make door tight fitting to prevent rodent entry. #49- 4-601. 11(c) : must clean grease and food debris build up from fryer unit and wheel bases. :#55- 6-201. 13 : must repair or replace damaged floor tiles throughout,stained & missing ceiling tiles in prep area and lobby. Must grout floor throughout including walk in cooler. #55-6-501. 13 : must remove standing water and keep floor dry. 6-202. 11 : must provide light shields at lights above cooking equipment and in prep area where needed. #57- 2-102. 13 : observed expired food handlers training for employees must provide and maintain.
