- comments: observed no hand washing sign at hand washing sink in kitchen. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed bulk containers not labeled in dry storage area. Instructed manager to label all bulk containers with common name of food.
38. Insects, rodents, & animals not present
- comments: observed over 100 mice droppings scattered on floor along walls and corners in prep, dish washing, dry storage and utility areas, on shelving unit under oven, on botttom shelves under paper goods, on bulk containers in dry storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued
45. Single-use/single-service articles: properly stored & used
- comments: observed single service articles stored on floor in rear storage area. Instructed manager to elevate all single service articles six inches off floor.
49. Non-food/food contact surfaces clean
- comments: observed accumulated grease buildup, food debris and spills on interior and exterior surfaces of steam tables, prep tables, cooking equipment, shelving units. Instructed manager to clean and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed leaking faucets on 3-compartment sink. Instructed manager to repair and maitntain.
55. Physical facilities installed, maintained & clean
- comments: observed grease buildup and debris on floors under all equipment, along walls and in all corners in prep, storage, utility and dish washing areas. Observed stained ceiling tiles and debris inside light shields in prep, storage, utility and dish washing areas. Observed spills on walls behind dish machine in dish washing area. Instructed manager to clean and maintain.
