- comments: a broken internal thermometer inside the walk-in cooler and missing internal thermometer in the three-door egg cooler. Must replace all.

- comments: instructed to replace the missing splash guard at the front prep hand washing sink next to the reach-in cooler.
43. In-use utensils: properly stored
- comments: ice scoop in the middle prep room must be stored in a clean container. Not on the top of the machine.
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: all stored items throughout the main entrance prep area must be elevated from the floors.

- comments: must not use milk crates as a means of elevation for stored items throughout the facility. Floors must be accessible for cleaning and monitoring at all times.

- comments: no chlorine chemical test strips for the low-temperature chlorine chemical sanitizing dish washing machine quaternary chemical test strips for the quaternary sanitizer being used at the three-compartment sink. Must provide to monitor sanitization levels. Priority foundation violation 7-38-005 citation issued.

- comments: all peeling plastic on all metal surfaces of equipment, coolers, fan guards etc. Must be removed ----all shelving and metal food cart in the rear garage storage room with excessive flour debris. Must clean and maintain.

- comments: noted outside food waste receptacle with bags of food waste overflowing the container and unable to close the container lid to be tight fitting. Must remove all overflowing garbage and, if needed, increase waste pick-up times or increase the number of containers needed to accommodate the amount of waste produced. Priority foundation violation 7-38-020(b) citation issued.

- comments: must remove all unused equipment, plastic crates, old furniture, and piled wood from the rear outside yard area to prevent rodent harboring.
