- comments: 6-501. 111 observed evidence of live insect activity on site. Observed about 10 live small flies on the walls by the front food prep area drink station and on the ceiling by the rear utility sink. Instructed to eliminate live small flies activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 15 observed the front entrance door and front entrance revolving door with a one inch gap at the bottom of said doors. Instructed to make said doors tight fitting and rodent proofed.

- comments: 5-501. 17 observed no covered receptacle inside of the women's toilet room. Instructed to provide.

- comments: 6-501. 12 observed minor accumulated debris on the lower shelves of a prep table and by refrigeration unit condenser. Observed some minor debris on the floor by the dish machine and a black substance on the wall by the dish machine. Instructed to clean all.

- comments: 2-102. 13 observed no food handler's certificates available for inspector's review. Instructed to provide food handlers certificates for review at all times.

- comments: 2-102. 13 observed the city of chicago food service sanitation manager on duty without a allergen training certificate. Instructed to meet requirement.
