- comments: observed 30 live small flies at basement storage area and 10 live small flies in mens washroom. Instructed manager must remove all insect activity by date of reinspection. Priority foundation 7-38-020(a).

- comments: 4-door prep reach in cooler door rubber gaskets ripped. Must repair/replace damaged door gaskets.

- comments: backflow prevention device not located on ice machine water line. Must provide/make visible a backflow prevention device for ice machine.
