- comments: observed no soap at the handwashing sink located in the kitchen prep area. Instructed management all handwash sinks must be supplied with soap at all times. Priority foundation 7-38-030(c). Citation issu

- comments: observed no paper towels at exposed hand washing sink in kitchen prep area. Instructed to maintain paper towels at all exposed hand washing sinks at all times. Priority foundation violation 7-38-030(c). Citation combined with above violation.

- comments: observed excessive black and pink slimy substance build up on inside interior panel of ice machine and dripping onto ice for human consumption. Must remove build up, clean, and sanitize. Priority foundation 7-38-005

- comments: observed frozen shrimp thawing in a container in the food prep area. Management instructed to thaw tcs foods under cold running water, in the refrigerator or in the microwave as part of a continuous cooking process.

- comments: observed seasonings and rice out of the original containers without labels containing the product name. Instructed to label all foods and spices with common name that are out of the original containers.

38. Insects, rodents, & animals not present
- comments: observed the rear exit door not completely rodent proofed as required. Must completely rodent proof door by sealing 1/4 gap at the bottom of the door.

41. Wiping cloths: properly used & stored
- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store in sanitizing solution between uses.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted aluminum foil being used to line on grease trap inside the kitchen area. Instructed management to remove cardboard and provide a smooth, cleanable, and non-absorbent surface.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no working sink stoppers on site at time of inspection. Instructed to provide adequate sink stoppers to fill 3 compartment sink.

- comments: found broken floor tiles behind the bar area. Instructed management to repair broken tiles and maintain.
