2. City of chicago food service sanitation certificate
- comments: no valid city of chicago certified manager certificate posted, nor manager on site while handling tcs foods (shrimp, rice). Instd to have original certificate and mgr on duty at all times. #7-38-012 priority foundation.

16. Food-contact surfaces: cleaned & sanitized
- comments: interior chute of ice machine and surrounding surface soiled with excessive black substance. Ice is used for human consumption. Instd to dispose of ice, clean and sanitize ice machine and maintain. Citation issued 7-38-005 priority foundation

25. Consumer advisory provided for raw/undercooked food
- comments: missing consumer advisory with disclosure and reminder on the menu. Instructed to disclose on the menu which foods (ex. Steak) can be ordered under cooked/cooked to order (steak, hamburgers) and which foods are raw (sushi, shrimp, aguachiles) with an asterisk next to those specific items; and then by asterisking them to a footnote that states these items are served raw or undercooked or contain (or may contain) raw or undercooked ingredients and also a reminder statement such as 'consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions' priority foundation violation 7-38-005 citation issued

- comments: previous core violations from 2/3/23 report 2570746 not corrected: 41 4-901. 12: **c** wiping cloths, air drying location observed wiping clothes laying on the counter top and prep stations. Must properly stored when not in use in sanitizing solution. 47 4-101. 11(b, c, d, e): **c** characteristics- materials for construction and repair ceramic/clay drinking cups on site. Instd to ensure cups are lead free or lead safe levels and maintain documentation of such on site; or remove cups from premises 49 4-601. 11(c): (c) nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Observed storage racks in the prep and dish washing areas dirty. Must clean and maintain. 55 6-501. 12: **c** cleaning, frequency and restrictions floors and floor drains through out not clean, excessive debris, and heavily stained. Walls through out prep area with food spills, splatter. Instd to clean and maintain same. 55 6-501. 11: **c** maintained in good repair, facilities (facilities not in good repair) observed missing and damaged ceiling tiles in the rear prep and dish washing areas. Also cracked floor tiles in the dish washing area. Must repair and maintain. 57 2-102. 13: **c** food handler training (pa 100-0367 food handling regulation enforcement act) missing proof of food handler training for all employees. Instd to obtain training and maintain certification on site. Citation issued 7-42-090 priority foundation
