- comments: person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued -

2. City of chicago food service sanitation certificate
- comments: no original and valid city of chicago food service sanitation certificate posted nor provided on site for present manager on duty. Instructed to provide. A least one certified food manager must be on site when food is handled(cooked rice,cooked beef,shrimps,etc. ) priority foundation: 7-38-012, citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site. Priority foundation violation 7-38-010. No citation issued

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no procedure/plan for responding to vomiting and diarrheal events. Instructed facilty to develop and maintain a procedure /plan and to maintain any appropriate supplies on site. Priority foundation violation 7-38-005. No citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: hand washing signage is needed at three handwashing sinks. Instructed to provide.

43. In-use utensils: properly stored
- comments: must not stored in use knives between prep cooler and prep table inthe kitcken. Must be stored in a clean container between use, or provide a knife rack.

- comments: must not use tin foil as liner for cooking equipment. Must be smooth and cleanable.

- comments: leaking water pipe under three compartment sink(third compartment),instructed to repair

- comments: grease trap lid under the three compartment sink is dirty and rusty. Surface must be smooth,cleanable and non-absorbent

Must install back flow prevention devices on water intake lines on ice machine and on mop sink. #58) 2-102. 13 observed no approved allergen training in accordance with pa 100-0367 on site. Instructed any persons with the city of chicago food sanitation certificate to comply with allergen training and maintain. Priority foundation violation: 7-42-090, no citation issued
