- comments: observed timed faucet not functioning properly at hand washing sink in women washroom. Water must run continuously for at least 15 seconds before reactivating faucet.
38. Insects, rodents, & animals not present
- comments: observed over 60 mice droppings scattered on standing mixer, on floor along walls behind stove and under prep tables in rear prep area, on floor in dish washing area, under pallet in breakroom near women washroom , on electrical box on shelving unit along west wall in rear storage area behind breakroom. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed 1/2' gap between rear exit doors. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed single service articles stored on floor in rear prep area. Instructed manger to elevate all single service articles six inches off floor.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed door gaskets in poor repair and ice buildup in walk-in freezer. Instructed manager to repair and maintain.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no test kit or irreversible temperature indicator for high temperature dish machine. Instructed manager to provide test strips or irreversible thermometer for utensil surface temperature in hot water mechanical operations. Priority foundation 7-38-005 citation issued

- comments: observed food debris and spills on interior and exterior surfaces of fryer cabinets, prep tables, coolers, freezers, holding units, shelving units. Instructed manager to clean and maintain.

- comments: observed condensation dripping inside bucket in walk-in cooler. Instructed manager to repair drainline and properly dispose condensation.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in women washroom. Instructed manager to provide.
55. Physical facilities installed, maintained & clean
- comments: observed rusty surfaces and peeling paint on ceiling in rear prep and dish washing areas, peeling paint on walls in prep, storage and breakroom areas, missing and stained ceiling tiles in sales and storage areas. Instructed manager to repair and maintain. Walls and ceilings shall be smooth and easily cleanable.

- comments: observed food debris and water on floors under all equipment, along walls and in all corners, and floor drains in need of cleaning in prep area. Instructed manager to clean and maintain.
