- comments: noted no certified food manager on duty while tcs foods are prepared and served (chicken, eggs etc)or city of chicago sanitation certificate. Instructed to have the city of chicago certificate on site at all times. Priority foundation #7-38-012.

- comments: noted incomplete pest control log book. No inspection receipts (at least twice a year per code). On-site at this time of inspection. Last inspection receipt presented was 1/20/2020. Instructed to have a pest control log book on site at all times. Priority foundation #-38-020(c) no citation issued
39. Contamination prevented during food preparation, storage & display
- comments: food containers stored on floor inside walk-in cooler. Must provide proper storage or raised shelving to store food containers 6' off floor.
41. Wiping cloths: properly used & stored
- comments: noted dirty wiping cloths on prep tables at front and rear prep areas. Not properly stored. Instructed to properly store all washcloths in a sanitizing solution especially when not in use.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: noted wash rooms on premise with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women wash rooms and unisex wash rooms
54. Garbage & refuse properly disposed; facilities maintained
- comments: noted outside grease container and surrounding not maintained with thick grease on the ground around the grease container and on top of container. Instructed to detail clean and maintain grease container and its surrounding. Priority foundation #7-38-020(b).
55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and maintain floors especially underneath frying equipment with heavy grease accumulation, around the dishwashing area and utility sink and behind the ice machine area with dirt and debris along with wall base.
