- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)no citation issued)

- comments: observed several bags of bread and chips gnawed, adulterated with mice droppings and display for sale. Instructed manager to discard. All food displayed for sale shall be wholesome and in good condition. Priority foundation 7-38-005 citaton issued

38. Insects, rodents, & animals not present
- comments: observed an infestation of appx. 500 or more mice and rat droppings on dog food shelf,candy suckers,bread,can goods,snacks,chip display boxes,paper goods,water display,rear storage area-along wall bases next to mop sink,reach in cooler & freezer,next to and in between storage bins,on floor under storage shelves in between clutter,under stairs leading to basement. Scattered on floor under most shelving units on sales floor. Must remove droppings and clean and sanitize areas. (priority 7-38-020(a)(citation issued)

- comments: must clean debris build up and spills from reach in coolers and freezers,display shelves,cashier booths,service counter and defrost small ice cream freezer.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacles inside of unisex toilet rooms. Instructed to provide.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage dumpster lid missing and trash and debris on ground around dumpster. Must replace lids to be tight fitting. Remove trash and keep area clean. (priority foundation 7-38-020(b)(citation issued)

- comments: must repair or replace stained ceiling tiles at rear area of the store.
