- comments: observed no adequate supplies on site to properly respond to vomit and/or diarrheal events that can potentially occur inside of the facility. Instructed to provide. Priority foundation 7-38-005.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed milk crates used for elevation in walk-in cooler. Instructed manager to provide appropriate shelving for storage.

51. Plumbing installed; proper backflow devices
- comments: observed leak at bottom of 3 compartment sink wash solution compartment. Instructed manager to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle for feminine hygiene products in staff toilet room and girls toilet room. Instructed manager to provide and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed missing lids on outside dumpster. Instructed manager that lids must be tight fitting per city code to prevent rodent harborage. Priority foundation 7-38-020(b)

55. Physical facilities installed, maintained & clean
- comments: obsserved cracked /damaged ceiling and walls in main prep area over the 3 compartment sink. Instructed manager to repair and maintain.
