- comments: observed rodent bait pellets, blocks, and bags of rodent poison stored on the floor, on the storage shelves, on top of packaged food in the retail area and behind the meat counter. Management instructed to store rodent bait in covered, tamper-resistant bait stations. Priority 7-38-015. Citation issued.

- comments: observed a pest infestation throughout the premises. Observed over 300 mouse droppings on the floor throughout the premises and under and on the retail storage shelves and under the display coolers in the meat prep area and along the walls of the meat prep area, in the utility closet where the hot water tank is located. Also observed 10 live small flying insects in the food prep area. Management instructed to clean and remove all rodent droppings throughout the premises. Additional pest control service is needed to eliminate the pest activity. Priority 7-38-020(a). Citation issued.

- comments: observed no pest control log book on the premises. Premises has a contract from hansen pest control. Management instructed to provide a pest control log book from a licensed pest control company with all required documents. Priority foundation 7-38-020(c). Citation issued.

- comments: detail clean the storage shelves throughout the premises.

- comments: observed inadequate hot water provided on the premises. The hot water at the 3 compartment sink was at 63f. Management instructed to provide adequate hot water above 110f to wash dishes and utensils at the 3 compartment sink. Priority 7-38-030(c). Citation issued.

- comments: clean and maintain the grease trap to eliminate the sewage odor at the 3 compartment sink.

- comments: must provide a covered waste receptacle in the washroom.

- comments: observed garbage overflowing over the top of the garbage dumpsters and a large pile of garbage stored next to the dumpsters on the ground. Management instructed to provide adequate garbage pickup to prevent the dumpsters from overflowing. Priority foundation 7-38-020(b). Citation issued.

- comments: the floor for the walk-in freezer at the rear of the premises is not smooth and easily cleanable and is made of raw wood. Must provide a smooth and easily cleanable surface in the walk-in freezer. Must paint or seal the wood flooring at the crawlspace.
