- comments: no city of chicago certified manager on site while handling tcs foods (eggs, ham). Instd to provide city of chicago certified manager on site at all times. Citation issued 7-38-012 priority foundation.
10. Adequate handwashing sinks properly supplied and accessible
- comments: no hand drying provision at handwash sink in customers toilet room. Instd to provide and maintain. Paper towels were provided by end of inspection. Priority foundation 7-38-030(c) see above for violation issued.

- comments: the following tcs foods in large deep containers dated and cooked yesterday (10/1/23) at improper cold holding temperatures in cooler: cooked broccoli soup 52. 1f, cooked red salsa 54. 5f, cooked green salsa 66. 7f. All discarded and denatured at this time. Approx 20lbs $100 value. Citation issued 7-38-005 priority
25. Consumer advisory provided for raw/undercooked food
- comments: missing disclosure for consumer advisory on the menu for eggs. Instructed to disclose on the menu which foods ( eggs) can be ordered under cooked/cooked to order with an asterisk next to those specific items; and then by asterisking them to a footnote that states 'these items are served undercooked'. Provide this footnote above the reminder statement such as 'consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions' priority violation 7-38-005 citation issued

- comments: improper cooling of cooked soup, and salsa. Instd to properly cool: using shallow equipment, (1) arrange in equipment to provide maximum heat transfer through the container walls; and (2) loosely covered or uncovered if protected from overhead contamination during the cooling period to faiclitate heat transfer from the surface of the food

- comments: inoperable food temperature measuring device on site. Thermometer on site inoperable due to dead battery. Instd to provide same and maintain same (capable of both cold and hot temperatures). Citation issued 7-38-005 priority foundation 7-38-005

- comments: food being stored in cabinet underneath front handwash sink. Instd to store food properly to prevent possible contamination
