- comments: observed black slimy substances on interior water components of the ice machine,dripping into ice 'ice is used for human consumption'. Instructed management to turn off machine, remove ice and have interior of ice machine cleaned and sanitized and maintain. Priority foundation violation 7-38-005,citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 30 live small flies on the walls and ceiling at front food prep areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a), citation issued.

- comments: observed torn door gaskets inside of the walk-in-cooler door and baskin robbins 1 door cooler. Instructed to repair/replace the torn door gaskets.

- comments: instructed to detail clean the storage racks inside the walk-in-cooler.
50. Hot & cold water available; adequate pressure
- comments: observed no hot running water throughout the premises. Handwashing sinks water temperature measured at 72. 0 f-75. 5 f and 3-compartment sinks water temperature measured at 75. 6 f-77. 0 f. Management immediately notified technician for service. Instructed to provide hot water above 110 f at the 3 compartment sinks and minimum of 100 f at all hand washing sinks. Priority 7-38-030(c),citation issued.

- comments: instructed to re-grout floor tiles in front of the rear 3-compartment sink areas and maintain.

- comments: previous core violation not corrected from inspection report#2371030 dated:05/22/2020 #56 hood above the sandwich prep microware oven, with dust accumulated. Must detail clean,remove dust and maintain. Instructed to correct noted violation and maintain by 12/15/2021. Priority foundation violation 7-42-090.
