- comments: person in charge at the time of inspection did not have a city of chicago food service sanitation certificate posted on the premises. Priority foundation violation.
2. City of chicago food service sanitation certificate
- comments: no city of chicago food service sanitation manager on-site during the initial inspection. The manager was telephoned upon my arrival and arrived during the inspection approximately 30 minutes later. Reviewed that a city of chicago certified food manager must be on-site at all times when foods are being prepared and served. Priority foundation violation 7-38-012 citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no written employee employee health policy on site verified by any employees working on site during the inspection. Must provide a verified written employee health policy for all employees. Priority foundation violation 7-38-010 citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: interior of the ice machines ice bin needs cleaning.
36. Thermometers provided & accurate
- comments: missing internal thermometer inside the front prep dairy cooler. Must replace.
37. Food properly labeled; original container
- comments: all bulk food containers must be labeled with the product common name.
49. Non-food/food contact surfaces clean
- comments: exterior of the oven and the exterior door of the walk-in cooler with food debris. Must clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: excessive rust and debris in the floor drain under the three compartment sink. ----food debris throughout the floor of the walk-in cooler. Must clean and maintain all.
57. All food employees have food handler training
- comments: some employees on site noted without proof of food handler training or certificates. Must provide.
