- comments: menu items such as eggs and burgers are not disclosed/reminded with an asterix as indicated on the consumer advisory statement at the the bottom of the menu. Must disclose/remind such items on both the printed and virtual menus. Priority foundation violation.

- comments: all bulk food containers such as flour must be labeled with the product common name.

- comments: noted interior of the cooks line oven with excessive burnt food debris. Must clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: water-damaged ceiling tiles in both washrooms must be replaced. ----noted peeling plastic metal protecting on the metal wall of the cooks line. Must remove to be smooth and cleanable. ----observed pooling water in several areas of the basement storage areas. Must remove pooling water and maintain dry floors.

- comments: noted broken/inoperable lighting inside the walk-in freezer. Must repair and provide adequate lighting.
