2. City of chicago food service sanitation certificate
- comments: observed expired city of chicago food sanitation manager on site while tcs foods are being prepped (such as chicken). Instructed must have a city of chicago food sanitation manager on site at all times when tcs foods are being prepped/served. Instructed must post a valid city of chicago food sanitation manager certification in plain view of customers at all times. Priority foundation violation 7-38-012. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up procedure and supplies on the premises for vomit and diarrheal events. Management instructed to provide both. Priority foundation 7-38-005.

23. Proper date marking and disposition
- comments: observed ready-to-eat, tcs foods that are refrigerated over 24 hours not date marked to indicate the date in which food must be consumed or discarded. Management instructed to date mark all ready-to-eat, tcs foods stored under refrigeration for more than 24 hours. Management discarded all foods which were not date marked. Priority foundation 7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 20-30 rodent droppings behind the cooking equipments in the prep area. Instructed to remove droppings, receive additional pest control service, and maintain premises free of pest activity. Priority foundation violation 7-38-020(a). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted milk crates being used as shelving and for storage throughout the kitchen. Must remove from premises and provide shelving that is free of unnecessary crevices and designed/constructed to be easily cleanable.

49. Non-food/food contact surfaces clean
- comments: noted heavy ice build up inside the chest freezers in the storage area. Instructed management to defrost, remove ice, clean, and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed no backflow prevention device. Instructed to provide.

55. Physical facilities installed, maintained & clean
- comments: observed accumulated food debris on floor under around and behind all cooking equiment and refrigeration units. Must clean and maintain.

- comments: observed inoperable ventilation in the women employee washroom. Must repair and maintain.
