2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site nor city of chicago food service sanitation certificate on premises while tcs foods (steak, chicken, etc. ) are being prepared and served. Instructed that certified food manager has to be on site at all times food is being prepared and served. Priority foundation violation 7-38-012. Citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed that several employees have not signed the employee health policy. Management instructed to have a signed copy for each employee. Priority foundation 7-38-010
5. Procedures for responding to vomiting and diarrheal events
- comments: person in charge unable to provide procedure for employee to follow when responding to vomit and diarrheal events. Must provide procecdure and vomit kit with the necessary items. Priority foundation violation 7-38-oo5.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs or posters near hand washing sinks throughout premises. Must provide hand washing signs and maintain.
41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on top of prep tables. Must properly store wiping cloths in a sanitizing solution when not in use.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical test strips on the premises to measure the concentration of sanitizer at the 3 compartment sink. Management instructed to provide. Priority foundation 7-38-005. Citation issued
51. Plumbing installed; proper backflow devices
- comments: additional water pressure needed at the hand washig sink in the employees washroom. Must repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed grease and dirt build up under around and behind all cooking equipment. Must clean and maintain. Must maintain floor dry at all times in rear food preparation area.
