- comments: observed 4 live cockroaches on wall in between reach-in-freezer and prep table directly across from the kitchen cooking stove. Instructed to detail clean and remove evidence of pest activity and sanitize affected areas;additional pest control service is now reccomended. Priority foundation violation 7-38-020(a),citation issued.

- comments: observed a torn door gasket inside of the 2 door prep cooler. Instructed to replace the torn door gasket.

- comments: instructed to detail clean the cooking stove/oven, prep tables, food storage containers,interior and exterior of the freezers and prep coolers,kitchen storage racks,steam table, and deep fryer.

- comments: observed water leak at base of faucet of the 3-compartment sink. Instructed to repair plumbing leak and maintain.

- comments: instructed to detail clean and organize rear dry storage areas and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning.

- comments: instructed to detail clean kitchen exhaust hood/filters to remove accumulation of hanging grease build-up and maintain.
