- comments: observed hot tcs foods at improper hot holding temperatures. Found beef gyro meat at 124f and sliced chicken at 89f in the steam well in the front prep area. Instructed management that all hot-holding foods must be at 135f or above at all times. Priority violation 7-38-005. Citation issued. Food discarded.
22. Proper cold holding temperatures
- comments: found cold tcs foods at improper cold holding temperatures inside the prep cooler in the front prep area. Sliced turkey, sliced ham, sliced capicola, tuna salad, mozarella, cream cheese, and garden cream cheese are found at temperatures ranging from 45f to 52f. Instructed to discard and denature said tcs foods and always maintain all cold-holding tcs foods at or below 41f. Priority violation 7-38-005. Food discarded. Refer to violation #21 for citation.
38. Insects, rodents, & animals not present
- comments: found an incomplete pest control log book. Observed no recent inspection reports (at least twice a year per code) on-site at the time of inspection; last report from smithereen pest control on site is 2/14/2022. Instructed to have a pest control log book on site all times with recent pest control reports. Priority foundation 7-38-020(c). Citation issued.
40. Personal cleanliness
- comments: observed all food-handling employees preparing food without hair restraints. Must provide and maintain.

- comments: observed a food handler without food handler training. Instructed management that all employees who handle/prepare food and who do not have a food manager certificate must complete food handler training and keep on site.

- comments: observed the city of chicago food manager without allergen training. Must complete training and keep certificate on site.

- comments: observed a previous core violation from 6/22/22 report #2559681 not corrected: violation #47 (4-501. 11) observed rusty shelves in the walk-in cooler. Must repaint or properly seal. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
