- comments: observed no city of chicago certified food manager on the premises while tcs foods (shakes containing dairy) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

- comments: observed no employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation 7-38-010. Citation issued.

- comments: found no recent pest control report inside the pest control log book; last report is from 12/10/2019. Instructed management that premises must be serviced at least twice a year per code, and reports and all other necessary documents must be kept on site at all times. Priority foundation 7-38-020(c).
